It is a mod for HL that was designed to be the most sadistic and evil game ever made and it pretty much lives up to that claim. It featured a tutorial where swedish voice-overs berate you for doing the tutorial, a single player campaign that featured black and some shades of gray, a level where you had to wait almost an hour for a train to arrive and a final level that i'm fairly certain you can't beat unless you cheat and if you cheat in the game you are killed for cheating after beating it.
But don't take my word for it.

So yeah, if you really want to make this game awful play this first.
No, that ruins the spirit of the game. No modes, just a bus that drives across a desert without anything to look at, and the only reason it drifts off to the right is to make sure that the player is actively playing for those 8+ hours.
This is also a sadistic games.

How about have a quick save, but no quickload.
I solved that by not having any of those. If you add anything it really detracts from the original idea.
How about the game allowing you to quickload to where you were after driving the same distance as you had driven at the quicksave?

edit: played the game. It didn't respond to me hitting the left key...or at least did so very slowly. Not sure if it is supposed to be poor controls (probable) but I guess because it is drifting right it seems like it is responding more to the right turn.
Many games automatically save for you at the beginning of a level, right? How about a single-slot autosave, that occurs at the beginning of every drive. You can quick-restore any time you want, but it always returns you to that initial save at 0 mph.
Better yet, you should be able to look to your left, right, up, and down. There should be absolutely nothing of note happening, no matter how hard you look.
Or even better, you should be able to turn around and look at what is going on in the bus. But nothing is going on as nobody else is riding the bus. It is only you for absolutely no reason whatsoever.
At night, you should be a little bit delusional in-game from the boredom. Something really subtle that rarely ever happens. Maybe weird lights in the sky.
1. No need for camera tricks. The game will do that to you as it is.
2. You can turn around and look at what is going on in the bus, but like looking down at your map, you can't steer your bus in the meantime, so you can't do it for very long. All you see are a bunch of people sitting there and doing nothing. Occasionally someone will scratch their nose or sniff.
3. Nothing special should happen at night. Just that you are now driving your desert bus at night. Occasionally you'll see a tumbleweed drift past, or a wisp of wind blow by. This should only happen once every hour or so.
4. The crash animation should be the most boring crash animation ever, so that you can't even have fun with the game by making your bus crash in interesting and hilarious ways. It doesn't matter what you do, you just see your bus emit a cloud of smoke and slowly drift to a halt.
# 2 does not seem in the spirit of the original. No companions. Just empty wasteland to keep you occupied.

This is the best idea I've read thus far
also: all the textures in the game should be different resolutions. Like, the steering wheel is this insanely detailed, bump-mapped and self-shadowing object, but the dashboard is a big 32x32 compressed .jpg. If you look back at your passengers, their eyes are highly detailed but they've got goldeneye-level faces.
Object occlusion shouldn't work. So, you can see Las Vegas hundreds of miles in the distance. And the sun through the ground.
The game should also crash for no reason sometimes.
Good point: your field of vision should be limited to the front 180 degrees of the bus, and all you see in the rear view mirror are the tops of peoples' heads, if that.
Other ideas:
The Loading Screen animations, if any, should give "tips" that have nothing to do with the gameplay, like: "Use the iron sights on your Desert Eagle to shoot the terrorists accurately in the head when they attempt to capture the bus," and "Save your Nitro for getting away from cop cars."
It bobbles for the first minute and a half of gameplay. Then it gets stuck to one side.
I mean, the road isn't perfectly paved, so I'd expect some sort of turbulance on the ride.
(and a shaky screen, however miniscule the tremoring, would piss me right the fuck off)

Frankly, I think having passengers even exist in the back is too exciting for the game.

The original sound effect from the game grates on your mind endlessly, but is useful. It tells you when you are about to run off the side of the road. It's a DIFFERENT kind of grinding, annoying sound.

It's insane monotony. Nothing, and I mean nothing, should make it more interesting. No people, no music, no little hoola girl on the dashboard. The idea behind it is not to be annoying, not to make the player frustrated or feel hatred, but to be the very essence of monotony. It's not exciting, but it's not frustrating. You are simply driving a bus across the desert. For 8 hours. And the bus veers off to the right a little bit. Nothing sadistic, nothing to make the player think violent thoughts. Just monotony that eats away at you because it's so monotonous.
How is a little bobble hula dancer doing the exact same motion for 8 hours not monotonous?
Also, I think both cities should give off an intense aura of fun, giving the impression that you could actually enjoy yourself if only you could get off the bus before it drives off into the boring, monotonous desert.

You wouldn't be able to run away from the police or anything. You would immediately be pulled over. The point of the no speed limit would be to prevent people from keeping the finger on the acceleration button.
