The stylus itself probably won't scratch the screen unless you're being really violent. Getting dirt between the stylus and the screen might do it, though. Took me 1½ years or so before I saw a scratch in mine, not too bothered about it though since it's only visible under direct light.
No, you don't need a screen protector from regular use. If you're playing Meteos and pressing particularly hard during a frantic scratch-fest, it might be worthwhile, but the screen is damn nice.
I bought a screen protector when I bought Ouendan as well. The biggest issue, which was already brought up, is getting a bit of dirt on the stylus and having that scratch the screen. Personally, I can't even tell it's on there anymore, it doesn't affect sensitivity enough for me to notice and I find it allows me to play without worrying about a scratch.
Ouendan tore up my screen protector on one of the spinners (well not really, but it was a very visible circular shape. Dust conditions? I was on a bus). Made me glad I had a screen protector.
