Unlike LoTR, A Song of Ice and Fire isn't written in a oh-so dry way you want to pull your hair out. LoTR is a fantastic fantasy series, but it's just not terribly enjoyable to read. (Though in fairness I still haven't started the second book of the ASoIF series)
To be honest its always been pretty clear to me who the real "safe characters" are.
Most of the shock comes from people mistaking Martin's secondary characters for primaries.

It eventually annoyed me more than anything that instead of the whole "nobody is safe and good things do not always happen" thing that you might see at first, all I see him ever doing is the most horrible thing to the characters..
okay that was not very clear. What I mean to say is that nothing good ever happens in these books. I could definately see the reasons for a lot of these things, I mean people are dicks and bad things happen to good people. But holy shit... yeah
The whole Cersei/Jaime thing has been a really really big plot point that's spanned the entire series
It goes a lot deeper than "gratuitous sex scene"
Not really in the case of, say, Tyrion (he's the only other character I can remember that has had pretty frequent sexual exploits.) but overall I agree.
The sex in asoiaf is all story driven, it's not even erotic at all. It just kind of happens.
This isn't some Clan of the Cave bear crap. (Uh, I have heard it was mostly cave-man porn?)
The sex is never gratuitous. I don't think it even gets too descriptive or anything. It is just that the books are realistic. Guess what? People have sex all the time in the real world.
It reveals things about the characters and whatnot. Helps develop the plot.

My problem is authors who try to work sex into the plot just because it's sex.
