Overkill?

How exactly does this work? Mouseover text indicates that you get one when you get a perfect special attack. And, at first, this was how they racked up. But now that I've got 3 per player I can't seem to reliably get any more.

    Yay, another thread about it.

    You get it when you execute a perfect special attack that kills the enemy.

    Teamups don't count.
