Is there any online tool that is good for creating surveys with varying reponse types? Radio button, text, etc
Anything with a decent looking layout and not riddled with advertising (advertising is fine, just nothing ridiculous), and I can get the results in some sort of PDF, or something hard copy that I can keep...
The company I work for did some consultancy work for a large Police Force over here and part of that involved an organisational survey. The Police Force had a few internal issues they were resolving, one of which stemmed from a racist joke being circulated via email, a joke which originated from someone inside the Force and which was picked up by the media... resulting in lots of complaints etc.
Unfortunately, the word "monkey" suffered as a result of this 'joke', and was taken so completely out of context as to mean serious disciplinary measures towards anyone using the word, in a joking way of not. The word 'monkey' was verboten.
Guess which surveysite we were using when putting together their survey? ;-)

Professional level and very flexible.
