So my music folders are a mess; in switching between hard drives a few different times and using different programs to rip everything I've managed to get my collection sprawled haphazardly across a bunch of different folders on different drives, with about 5 different naming conventions.
Is there any program that will automatically sort everything out for me, or am I best off doing it all manually?
I use the Picard Tagger. It compares the waveform of your files against an online database and will automatically tag, rename and move the files (to your preference). If it's not sure about a file, for example if it comes up with multiple search hits, it will give you some options for what to tag it with. It will save you many hours of tedious work, but if your collection is very large expect to spend some time choosing tags for the files that don't get done automatically.

Seconding the MusicBrainz love. If you need to do any manual editing afterwards, you can use foobar2000 (which also happens to be an awesome dead-simple media player) or MP3Tag.
This probably won't help you a whole lot just working straight from scratch (crappy collection), but I use MediaMonkey (direct link to free download) for my music library management/playing needs. It's a bit like Amarok (Linux program), but I doubt you've used that. It has some nifty features for organizing songs. You can mass edit IDv3 tags, mass rename files based on IDv3 tags, mass change album art, get album information from Amazon, and some other stuff. Also it has Last.fm support which is pretty cool (actually I think that's in the newer beta, but whatever). I know this was slightly different than what you might have been thinking of but I find it works well once you already have a system going. It's a snap for me to add more music into my library, edit the stuff to conform with my current conventions, etc.

It's definitely not for everyone, and there are plenty of reasons not to use it, but have you considered iTunes? It's pretty good about organizing everything for you (which some people don't like) and you can consolidate your library to move all the scattered files into one place. If you don't like using it for a player, you can dump it after importing everything and if the ID3 tags are all correct, you'll have a nicely organized hierarchy with which to work.
