Amending Lease - Switching Tenants

My landlord and I have come to an agreement whereby my roommate and I would vacate our 2BR, and 2 new tenants would move in. To achieve this, we would like to amend our original lease, making the 2 new tenants the responsible parties. To make a long story short, this is preferable to canceling the current lease and then drawing up a new one from scratch.

What language should I use when drafting this lease amendment? Anything I should make sure to add/not add? The general idea is as follows:

"This is an amendment to the lease, dated XX, between XX (Landlord) and A and B (Tenants). Landlord and Tenants agree that, as of August 20, C and D (New Tenants) will take the place of Tenants on this lease. Tenants will be absolved of all legal responsibilities and liabilities pertaining to this lease. These responsibilities and liabilities shall be transfered to New Tenants."

I should probably flesh this out some. Any thoughts/ideas?
