Computer screen keeps going blank -- video card problem?

Hi, my computer screen keeps going blank in the middle of using the computer. I can't do anything to bring a signal back to the screen except reboot it, and then after a couple minutes the monitor will lose signal again. The computer also recently said it didn't recognize the video card and installed new drivers - that was a couple days ago, and now the problem with the screen going blank is becoming more and more frequent. I'm not even sure what kind of video card it is anymore, since its name doesn't show up in the device manager anymore.
So could this be a problem with the video card, and I need to replace the card? Or could it be something else?
Any help would be greatly appreciated.
