    it was the dead guy's own fault kinda according to the probe

    he didn't turn enough and wasn't experienced enough

    but the blame isn't entirely his
    not letting them actually practice on the track probably didn't help his chances

    from reading the article on the probe it was such a ridiculous one-in-a-million thing that they never really considered it. it really was a freak accident with everything culminating into what happened
