How do you not know what these things are?
Vulva are the... external bits; the outside edge, where there is still hair.
A douche is usually either a bag attached to a hose and a rubbery nozzle, or a little smaller, like those blue bulbs you use to wash your ears only a little different. There are special soaps you can use, and I would assume some could be flavored, I guess. The soaps are mainly designed to not cause irritation.
A douchebag, from my understanding, is the thing used to store the douche, and therefore sits on the sidelines and only watches, never getting any action. That would be my stab at the origins of the insult.
