Image upload bug

Been trying to upload photos taken on my iPhone (iOS 9.3.1) and every time I try it tells me the image is "already in the discussion and won't be uploaded" despite the picture only being a few seconds to minutes old and uploaded literally nowhere else yet.

Weirder still, I can work around this by waiting for iCloud to sync the photo to another device (iPad with same OS version) and upload it from there.

I've had this problem when I uploaded an image, then remove it and try to upload the same image, or an image with same name (like if I forgot to resize or rotate it or something). Is that something like what you're doing?

Nope, brand new image, taken on the phone and cropped ready for upload as much as a minute prior to the first attempt.
