Basically white has lost the game because black has 3 extra pawns, one of which is about to promote and the only way white can stop it is to give up her rook. White is about to resign, but decides to throw in a couple of checks first, and black responds by making the only move that can possibly lose the game, which is going behind the pawns. This leads to white checkmating black.
As for the chess notation: the rows are labeled 1-8, with 1 being white's back row, and 8 black's back row. The columns are labeled a-h, with a on the queen's side and h on the king's side (i.e. the white queen starts at d8, and the white king starts at e8). Each piece (but not the pawns) have a letter B: bishop, N: knight, R: rook, Q: queen and K: king.
A move is noted by putting the name of the piece, followed by the square it moves to: e.g. Re3 means the rook goes to the square e3. If more than one rook could go to e3, then the starting row, column or both row and column are used to disambiguate the pieces.
A capture is marked the same as a move, but with an x between the piece name and the ending square: e.g. Rxe3.
Pawn moves and captures are done similarly to piece moves and captures, except no piece letter is given (pawns don't have one), and for pawn captures the starting column is always given.
Some symbols are also added after the move: + denotes check, ++ double check, # checkmate. Other notation can also be added after the move: ? is a bad move, ?? is a horrible move, ! is a good move, and !! is an excellent move.
