Also, several other factors that make it so that there are relatively few advantages for Americans seeking health care in Canada. (Still costs money, no unique services, lower levels of specialization, etc.)

What you are likely seeing imo is that instead of the polls starting to shift back towards center after the initial bump, they are instead holding steady. Which is good.
When more polls come in, and if there is another bump from the last debate we may see some upward movement, but holding steady at this stage of the campaign is a very good thing.

I think there's been some pretty exciting individual polls. it's possible that at this point it just takes more to move 538? I haven't looked super close but I've come across several where she's ludicrously far ahead. also maybe she's just expanding a lead in certain sured up states and 538 is not impressed.

Second, you're now actually misrepresenting the typical motivation for medical tourism. Medical tourists don't travel because of cost issues; they do so because of quality and availability. Poor Americans were never going to come to Canada for health care; the question is whether Canadian facilities can offer high-tier, specialized services that would otherwise be unavailable to rich Americans (at a lower cost). UHN believes it can. Toronto's Sick Kids hospital actually does, though this isn't typically considered to be the same as other sorts of medical tourism, for obvious reasons.
