Most glorious Arthas game: I go all Tempest talents against a team with Butcher, Twin Blade Varian, and Greymane.
Everything else was tank to maximize the time I could spend tempesting.
You can imagine how that turned out.
Constant 60% reduction of about three-quarters of their total damage output.
How the fuck does Azmodan die 8 times to that comp? He should be functionality immune to you by virtue of simply out-ranging your damage. And if Illidan dives him, Azmo can just trade with his beam -- he actually beats Illidan in that matchup if he invests down that talent path.

One was when he blew his beam on a tower and I dove him.
Mid game was me taking the "dive to ally" talent and just diving out of range when he beamed me.
Or just ulting him to cancel his beam.

50% CDR from insight is a bit nice, but nothing I would think is worth a lvl 20.

It's very good for sustain, or if you expect the game to go long. Otherwise, I showed a while back in a post that you are better off just taking other talents to get additional damage if all you're trying to do is burst someone down.

You'd think so, but sadly he's just not great at it. I tried making a Kara heal machine using lots of CD reduction, but it just doesn't work. The biggest problem stems from having to go in to activate your CDs. Sure, when you're punching, your W/E are just up, but that doesn't happen consistently. People dash away or you get CC'd down.
Kara is best suited to a double heal comp. His weak AoE heals have a better chance to gap fill the other healer's downtime.

But he's even less of a dive assassin. If you're doing that, just abandon healing entirely and go with a real assassin.
I play him just to be a somewhat flexible sustain who helps himself and other stay in lane, push minions okay, can take little risks to not get caught out of position and win trades, and do some camps in a pinch if the rest of your team isn't on top of that.
Transcendence helps with all that way more. For Insight, I'm not a fan of quests in general because it's better to have a bonus now to snowball off of than a delayed bonus that may or may not pay off fast enough.

Which is why, in those cases, he usually wants iron fist. Oh and also 7SS. That's his big boi burst and very decent.
It's also that his healing from autos can't really be used to sustain between fights unless you're mindless in lane or on a camp. His damage isn't terrible, and he can secure kills. That's his niche: a healer that can secure kills if necessary. He's not great at either, but at higher MMR that's the function he's got. You're less likely to meat-grinder at that point where he can just auto the enemy team for heals. Being able to bop a Valla who is causing trouble is worth more than the small amount from the healing level 1s.
I think firestomp is the standard build now with spell shield at 1.
That's what I see most often at high levels. That spell shield cdr just makes him so damn tanky.

You're tanky, but your CC options suffer greatly. Only one charge of Q and W with no cdr. Once both are blown, your only presence is the weakening effect from flame. You're sort of a less useful E build Leo at that point.
The whole "Khara isn't a great Healer" thing is predicated upon the fact that they literally fucking increased his Breath of Heaven cooldown by like 50% because Iron Fists was so prevalent, and it didn't feel right for someone with so much offensive power to be such a good Healer (who the fuck is Rehgar, am I right?). He used to be an incredible Healer. He used to have amazing throughout potential.
But it's hard to be a good Healer when your only heal has a 10 second fucking cooldown.
All they ever had to do was have Iron Fists increase the cooldown of Breath of Heaven to 10 seconds. That's it. Leave the other options alone, and then you have options. Then you have multiple viable playstyles.
But Blizzard can't balance their way out of a fucking paper bag, so this is what we're left with.

The one and only time I would go for Iron Fists is on Immortal and Shrines, and even then I'd think twice.
